Did you take a look at the link for the M1 form ? That will be what they send you I would think.. if so take it to your doctors .. I just strolled in to my doctors with mine I did not make an appointment.. ask to see the doctors secretary as it's them that releases the info back to the DVLA not the doctor personally.. but they will have been written too also and may well have already released what info they have or what was requested .. my advice is try and mirror yours to as close as possible to there's as it's an offence to knowingly or not declare .. but obviously you don't want to declare something that may have no relevance .. I wrote a letter and attached it to the M1 .. you can also submit forms via email now as well as postal service.. I have been doing each reply by both methods as they can cancel the application if paper work is not present or to late .. the email address isWhen I went for my blood test I was told to take doctors details list of medication & name of any specialists I was under, all original documents
I got there and he said my Secretary normally deals with the paperwork. I got all the paperwork out and was told he didn't need it when I filled in my details he stoped me when I got to my doctors details, we then began answering the questions and I said about antidepressants and offered him my list of medication he said no he would just put the antidepressants down. This doesn't fill me with confidence it must seem to them that I'm not telling them everything
