No D27 form received

Hey guys,

I should have received the D27 form about a month ago but I haven't just yet. I've decided to try and get hold of the D1 form instead but I'm unsure of what to actually apply for. I was disqualified and told I had to take an extended driving test to get back on the road, so does this mean I have to apply for a provisional as if I was a first time driver? Or do I apply to renew after disqualification? I just don't really wanna apply for the wrong license, send the wrong fee and have this all take a lot longer than it should.
